Their offerings include inpatient psychiatric care, outpatient therapy, addiction recovery services, medication management, and integrated primary care for individuals, families, and communities.

The Project
4C Health partnered with Project Matters to explore the need for a 23-hour Crisis Stabilization Unit (CSU) and identify potential barriers to bringing it to life.
To uncover the full picture, Project Matters conducted stakeholder interviews across six counties—including emergency department staff, law enforcement, and mental health providers—and led focus groups with 4C Health staff. These conversations revealed key insights around service gaps, referral patterns, impact potential, and systemic challenges.
Findings were paired with research on comparable models and compiled into a comprehensive report designed to guide the path forward.

This collaborative and insight-driven process helped launch a groundbreaking pilot project: 4C Health’s 23-hour Crisis Stabilization Unit, now operating as 4C Psychiatric Urgent Care.
The facility provides short-term crisis intervention for individuals experiencing behavioral health or substance use emergencies. Today, it not only serves communities across North Central Indiana—it’s become a model for rural crisis care statewide.
